Gracey care is a virtual clinic specializing in Care Management for medically complex seniors aging at home. We partner with Home care agencies and Senior leaving communities to deliver Medicare reimbursed clinical oversight, combining on-site caregiver support with remote care coordination under the clinical supervision of Gracey's, medical director and billing providers. Our core services span three Medicare program: (1) Principal Illness Navigation (PIN) provides person-centered assessment, SDOH support, and navigation services for patients with serious illness, delivered by trained on-site caregivers and Gracey's remote care team. (2) Chronic Care Management (CCM) delivers structured care coordination for patients with two or more chronic conditions, including care planning, medication management and provider communication. (3) Principal Care Management (PCM) manages patients with a single high complexity chronic condition. Already credentialed with Medicare and Medicaid, Gracey Care is actively pursuing credentialing with additional insurance carriers with a goal of extending our chronic care management services to a broader patient population.
